- COMPOSITION:
Morphine hydrochloride 10 mg/1 ml; 20 mg/1 ml in one ampoule
ACTION:
Principal alkaloid of opium and representative of the group of
narcotic analgesics. It possesses a strong analgesic action
connected with influence on specific opium receptors in CNS. It
exercises a marked analgesic effect which does not affect the
consciousness and motor functions. Besides the symptoms of CNS
suppression, morphine provokes stimulation of some hypothalamic
centers: appearance of hyperglycemia and antidiuretic effect.
Bradycardia may also occur. The excitability of respiratory and
cough centers decreases, as well as pulmonary ventilation. The
higher doses provoke drowsiness. The motility of the smooth muscles
of the intestines decreases, with a parallel rise in the tone of the
sphincter, leading to constipation.
PHARMACOKINETICS:
Morphine metabolizes by demethylation and glucuroconjugation. It is
excreted mainly with the urine in conjugated form and a small part
of it - in unchanged form with tears, breast milk, gastric and
intestinal juice, and urine. It undergoes enterohepatic turnover.
Oral absorption is influenced by many additional factors, hence oral
administration is limited. The action of morphine starts 10-15 min
after subcutaneous injection and lasts for 3-5 h for a single dose.
INDICATIONS:
Severe pains (malign tumors, heavy trauma, surgical interventions,
myocardial infarction), in cardiac asthma, as well as for
premedication. It is rarely used in bad cough or in radiological
examinations of the gastrointestinal tract.
CONTRAINDICATIONS:
Children aged below 5 years, elderly and very weak people, in cases
of insufficiency of the respiratory center, alcoholic psychoses,
pregnancy (morphine passes through the placental barrier).
In injection form - in anoxia, lung edema, broncholitis, cerebral
edema, myxedema. Morphine should not be applied in case of
"acute abdomen." |
SIDE EFFECTS:
In case of acute intoxication, death is caused by paralysis of the
respiratory center. Nausea, vomiting, constipation, pylorospasm,
dysuria, allergic reactions.
PRECAUTIONS:
Atropine is applied subcutaneously to curb the side effects of
morphine.
DRUG INERACTIONS:
Potentiation of some effects may occur due to interaction with
alcohol, general anesthesia, sedative and hypnotic drugs,
neuroleptics, antiepileptic and antihistamine drugs. Alkalizing
drugs reduce the excretion of morphine with the urine.
DOSAGE AND ADMINISTRATION:
Adults: subcutaneously 1 ml of 1 % solution (= 0.01), the first
injection - with reduced dose.
MEDICINAL FORM AND PACKAGE:
Ampoules of 10 mg/1 ml (1 %) and 20 mg/1 ml (2 %) in packs of 10 and
50.
STORAGE:
At moderate temperature (15-30 C) in a place protected from direct
sunlight.
EXPIRY:
3 (three) years.
