EB0280-424-08 DIGITALIN VIAL 10 ML 1 % SOLUTION

COMPOSITION:
Digitoxin 1 % - 10 ml.

ACTION:
A preparation with a potent cardiotonic action. It is the most active glucoside of Digitalis purpurea. Digitoxin enhances the systole, prolongs the diastole and regulates the cardiac rhythm. It restricts the blood vessels in the abdominal cavity, improving coronary blood flow, eliminates the congestive phenomena in cases of cardiac decompensation, and regulates the circulation. Digitalin is the most toxic digitalis preparation, owing to its considerable content of digitoxin.

PHARMACOKINETICS:
Its action begins 30 min after administration, the maximum effect comes after 4-12 h. the preparation is eliminated through the liver and is characterized by a small therapeutic range and symptoms of cumulation. The preparation is quickly and almost fully absorbed by the gastrointestinal tract. Its absorption is facilitated if the preparation is taken with food rich in fats.

INDICATIONS:
Chronic cardiac insufficiency of II and III degree with different origin - mitral valvular disease, hypertension, myocardial dystrophy, myocardial sclerosis and tachyarythmic form of atrial flutter and fibrillation.

CONTRAINDICATIONS:
Absolute contraindications - venticular tachycardia ( it may turn into venticular flutter) and digitalin intoxication. The application of the preparation is not advisable in cases of pronounced bradycardia (under 50 beats per min), conduction disturbances and full atrioventicular block, heavy stenocardia, insult, pulmonary embolism and renal insufficiency.

SIDE EFFECTS:
Anorexia, nausea, vomiting, hemorrhagic lesions of the gastric and duodenal mucosa, diarrhea, slight disturbances, bradycardia, polytopic extrasystoles, AV-block, coronary nspasm, reduced diuresis, headache, aphasia, hallucination, gynecomastia and eruptions.

PRECAUTIONS:
At appearance of toxic symptoms during the treatment, the administration of digitalin is discontinued, and atropine and potassium preparations are administered. The transition from digitalis therapy to treatment with another cardiac glycoside must be made several days after digitalin has been stopped, and under ECG-control.

DRUG INTERACTIONS:
The action of digitalin increases under the influence of diuretics, calcium and magnesium preparations, reserpin, sympathomimetics and parasympathicolytics. Probenecid and etacrinic acid inhibit the resorption of digitoxin in the small intestine. It is not advisable to combine the preparation withtolbutamide and clofibrate, which are bound to the plasma albumins and thus increase the elimination of digitoxin from the organism, which makes the choice of adequate doses difficult.

DOSAGE AND ADMINISTRATION:
Digitalin is applied orally. It is dosed individually, depending on the nature and the degree of insufficiency. For slow digitalization digitalin is applied in doses of 5 drops before the meals, 3 times daily for 4-6 days. In higher cases 75-90 drops are usually enough for a treatment course, in more severe cases - up to 100 drops. The full saturating dose of digitalin is 1.5 to 2 mg. The supporting dose is from 0.1 to 0.2 mg daily, this dose being applied during a long period with pauses of 2-5 days.

MEDICINAL FORM AND PACKAGE:
1 % solution in vials of 10 ml.

STORAGE:
At moderate temperature (15-30° C) in a place protected from direct sunlight.
