COMPOSITION:
Isosoebidi dinitras 10 mg/10 ml in one ampoule.

ACTION:
Antistenocardic drug from the group of nitropreparations.
Isodinit dilates the coronary vessels, and to a lesser degree also the other vessels in the organism, which leads to decreased peripheral vascular resistance and arterial pressure: by decreasing the tone of the venous system and the blood flow to the heart, it exercises a favourable influence on pre-load of the right half of the heart. The effect of isodinit is more prolonged than that of nitroglycerine.
It has a high affinity to the hepatic nitarte reductase, which converts it into nitrate reacting with SH-groups in the vascular walls, and the S-nitro thioles obtained activate guanilate cyclase. This leads to cumulation of intracellural cyclic GMP and to inhibition of the calcium-dependent electromechanical coupling of the vascular myocytes. Decreased concentration of the free intracellural calcium ions is observed. The tension of the ventricular wall decreases which leads to a decrease of the oxygen consumption in the myocardium. The preparation improves mainly the irrigation of the subendocardial layer of myocardium.
Isodinit metabolizes in the hepatocytes up to active metabolites: mainly to isosorbid-5-mononitrate and less - to isisorbid-2-mononitrate.

INDICATIONS:
Isodinit is applied first of all both for arresting a stenocardic attack and for prolonged treatment of a stable angina pectoris during exertion and rest; in spastic diseases of the peripheral vessels - obliterating endarteritis, angiospastic retinitis, etc.

CONTRAINDICATIONS:
Acute myocardial infarction with arterial hypotension, cerebral hemorrhage, increased intracranial pressure, glaucoma, shock and collapse conditions.

SIDE EFFECTS:
Headache (up to 20%), dizziness, weakness to which the patients get quickly accustomed.

DRUG INTERACTIONS:
The hypotensive effect is increased upon simultaneous administration of antihypertensive drugs, vasodilators and diuretics (especially in aged patients).

DOSAGE AND ADMINISTRATION:
Intravenously:
From ampoules for infusion with solution prepared extempore; the dosage is individual according to the patient's condition and the gravity of the cardiac complaints, but it should not exceed 2-10 mg/h.

MEDICINAL FORM AND PACKAGE:
Ampoules of 10 mg/10ml in packs of 50.

STORAGE:
At moderate temperature (15-30° C) in a place protected from light.

EXPIRY:
Two (2) years.
